Scientific name: MUC4 Immunohistochemistry Test
Immunohistochemical assay for MUC4 (Mucin 4) aims to detect this membrane-associated mucoglycoprotein. MUC4 is a highly sensitive and specific marker for synovial sarcoma (SS), being expressed in >90% of cases regardless of histological type (monophagus, biphasic). It is also expressed in many adenocarcinomas, particularly of the pancreas and lung.

- There are no numerical values. The tissue expression is read qualitatively:
- Positive (strong and pathological): Strong and widespread cytoplasmic and membranous staining in the majority of tumor cells. This is the typical pattern in synovial sarcoma.
- Negative or weak: absence of staining or weak, mottled staining. Strong, homogeneous cytoplasmic staining in a fusiform tumor is the definitive indicator.

Conditions & Notices
- Fasting is not required, as the sample is a surgical tissue sample.
- The tissue sample must be immediately preserved in 10% formalin.
- This analysis is routinely requested when synovial sarcoma or any nonspecific spindle-shaped soft tissue tumor is suspected.
- It is often requested as part of a diagnostic package for spindle cell soft tissue tumors, with markers such as TLE1, EMA, CD99, and BCOR. FISH testing for t(X;18) chromosomal translocation is strongly recommended, either concurrently or subsequently, to confirm the diagnosis.
