The anti-endomysial antibody (IgG) test is an immunological test that detects the presence of IgG autoantibodies directed against endomysial protein (the connective tissue surrounding smooth muscle fibers). This test is used in a limited, complementary manner, primarily in patients with selective IgA deficiency, a condition present in approximately 2–3% of celiac disease patients. In these patients, IgA antibody tests (such as tTG-IgA and EMA-IgA) are false negatives, so IgG tests (such as EMA-IgG and DGP-IgG) are required to reach a definitive diagnosis.
