The uterus is divided into four imaginary quadrants. Ultrasound measures the deepest vertical pocket of fluid in each quadrant. The measurements are summed to calculate the total Index (AFI).

If the abdomen size is smaller or larger than expected for gestational age. High-risk pregnancies or maternal diabetes. Decreased fetal movement reported by the mother.
