Immunohistochemical assay for H-Caldesmon (Heavy-Caldesmon) aims to detect this protein, which binds to actin and actomyosin in differentiated smooth muscle cells. It is a highly specific marker of smooth muscle differentiation, as it is expressed in blood vessels, the uterus, the gastrointestinal tract, and tumors arising from them (such as benign and malignant leiomyomas). It is negative in gastrointestinal stromal tumors (GISTs).
