An angiotensin-converting enzyme (ACE) test is a quantitative measurement of the serum level of this enzyme. ACE is primarily produced in the endothelial cells of the lungs and kidneys and plays a key role in the renin-angiotensin-aldosterone system (RAAS) by converting angiotensin I to the potent angiotensin II. Additionally, it is produced in large quantities by epithelial cells (monocytes) in granulomas, which are small masses of inflammatory cells characteristic of diseases such as sarcoidosis. Therefore, blood ACE levels are often elevated in the presence of active granulomas.
