The 11-deoxycortisol (11-deoxycortisol) or compound S test is a quantitative measurement of the serum or plasma level of this steroid intermediate. Compound S is produced in the adrenal cortex and is a direct precursor to cortisol, which it is converted to by the enzyme 11-beta-hydroxylase. The test is primarily used to diagnose 11-beta-hydroxylase deficiency, a form of congenital adrenal hyperplasia (CAH), in which this deficiency leads to an accumulation of compound S and its precursors and impaired adrenal hormone production.
